Our Targeted Case Management (TCM) services are designed to help individuals with mental health needs gain access to essential medical, social, educational, and community-based services. Through personalized planning and coordination, our Case Managers ensure that each recipient receives the right support at the right time.
We work closely with clients and their families to identify needs, set goals, and connect them with resources that promote independence, stability, and overall well-being. Whether it’s healthcare, housing, employment assistance, educational opportunities, or social support, our mission is to optimize the individual’s functioning and quality of life through effective coordination of care.
Our TCM services are individualized and client-centered, ensuring that frequency and type of support reflect the specific needs, goals, and abilities of each recipient—not simply what is covered by Medicaid or other funding sources.
Who provides Targeted Case Management
Targeted Case Management services are provided by qualified professionals with the necessary training, knowledge, and credentials to coordinate complex care needs. Our Case Managers are experienced in navigating community resources and working as part of a larger interdisciplinary team that may include psychiatrists, psychologists, counselors, nurses, and social workers.
Personnel qualifications include:
A Bachelor’s or Master’s Degree in Social Work, Psychology, Counseling, or a related human services field.
Relevant State of Florida registration or license as required by law.
Training and experience in mental health services, community resource coordination, and client advocacy.
What Targeted Case Management Includes
Our TCM services cover a wide range of supports to ensure continuity of care and promote client independence. These may include:
Assessment and Planning – Identifying individual strengths, needs, and goals to create a personalized care plan.
Coordination of Services – Linking clients with healthcare providers, mental health professionals, housing services, educational programs, and community resources.
Advocacy – Supporting clients in navigating systems and ensuring their rights and needs are respected.
Monitoring and Follow-up – Regularly reviewing progress, updating care plans, and adjusting services as needed.
Skill Development Support – Assisting clients in learning to access resources independently and advocating for themselves.
Family and Caregiver Support – Providing education and guidance to families or caregivers to better assist their loved one.
